I occasionally get the same issue unfortunately. Building or saving the file often sorts it out though. Having said that, if I invoke a shortcut key to pop-up the list, the pragma marks are almost always in the wrong place or not displayed - most unfortunate, as it means they are useless unless you use the mouse (which I try to avoid where possible).

Hopefully it's fixed in Xcode 3.
