I observed a behavior like the one you are describing with Xcode 2.x: I added a manual copy phase to my target. Xcode compiled the copied code and changes made in the editor window belonging to the debugger ended in the copied files. A new "build" copied the old, non-changed files over the modified ones. I took me quite some time to sort out why all of my changes weren't in the original files...
