I'm on 10.3.9 and Xcode 1.5 (IDE 389.0) and Safari 1.3.2
so what I'm seeing may not be applicable.
Anyway, it often happens that when I try to click on
part of a window of a different app, it often seems
to make the formerly current app not so, but doesn't
make the clicked-on window's app really active, either.
Often it doesn't bring it to "front".
Sometimes, clicking on the icon in the dock solves it.
Usually, clicking on the title bar, or the bottom left
border, completes the change-over and then I can click
icons and buttons, select text, etc. It happens more
often when switching to Adobe Reader or Safari than
when switching to Xcode, and don't recall it happening
when switching to Finder.
