In addition to all the maddening window activation problems* Mac OS X has, in general (I think since Tiger, at least), Xcode sometimes does some especially erratic things.

Just now, I was in Terminal.app, I click on the dock icon for Xcode, and I saw something flash out of the corner of my eye. I think it was the project window (I use the condensed view) disappearing. The menu bar still belonged to Terminal.app. One source file is still open. If I click (with another app frontmost) in the window anywhere but the title bar, the click is ignored. If I click in the title bar, the window comes forward, but the adornment is not redrawn to indicate an active window. The previously-active window appears properly deactivated.

No matter how I attempt to activate Xcode, its menu bar never returns, and I can't quit the app.

Anyone else seeing this?

I had this happen to me the other day, but the menu bar actually disappeared altogether, and I had to restart the machine.

* I'm sure you've all seen it: you click in another window, and the click is seemingly ignored; two windows will be active at the same time (in the same or different apps); the non-frontmost window will still have keyboard focus (same or different apps)...

Yes, but you get used to first clicking in the desktop and then in the window you want to activate after a while - it just becomes second nature.
