Do you reproduce it with using qlmanage on other kind of files like doing something like:
"qlmanage -t /path/to/some/other/file"

I would, but I have no idea how to reproduce the issue outside of my environment.
I'd be curious to know exactly how you invoke qlmanage under Guard Malloc (and with what exact version of the OS) as I can't reproduce that problem.
I followed the instructions in the developer documentation here: <file:///Developer/Documentation/DocSets/com.apple.ADC_Reference_Library.CoreReference.docset/Contents/Resources/Documents/documentation/UserExperience/Conceptual/Quicklook_Programming_Guide/QLDebugTest/chapter_11_section_2.html>
Then I pointed it to a file I wanted to look at, and set the executable as the current executable, and then enabled Guard Malloc in the Run menu. And I'm running 10.5.4 on an Intel Mac.
