I've seen this same problem. I don't know exactly what fixes it, but
I have managed to get it to go away.
When I can't get it to go away I can work around the problem by
pausing the app and going into gdb and setting future break points.
This allows the break points to be resolved properly.

What is going on? How do I escape this? Debugging with print
statements is NO fun.
