I used to have the "yellow breakpoints get ignored" problem. I'm
trying to remember what fixed it. I found the solution in the list
archives somewhere, but can't find it quickly now.
Try turning of "Load Symbols Lazily" in the Debugging tab of Xcode
Prefs.

I played with Activate/Deactivate breakpoints button, it helped.

Xcode 3 has started to not be able to stop at breakpoints I have
set. It turns them yellow and "ignores" them.
Sometimes saving the file that's open in he editor gets around
this. Sometimes quitting and restarting does but neither is reliable.
What is going on? How do I escape this? Debugging with print
statements is NO fun.
