This is the "automatically attach on crash" feature found in later
versions of Xcode. Like you, I find that this feature works fine most
of the time, but perhaps not always. When it doesn't work I usually
launch my executable in the debugger directly (Cmd+Y), instead of
relying on it to "attach on crash". This seems to work for me as a
workaround.
If you can reliably reproduce a problem with the "attach on crash"
feature running the latest version of Xcode (3.1), you should file a
bug report with Apple:
