Check your logs. I've had similar problems with the dock. I
terminates an App, but it's still flagged as running in the doc.
The last time I checked the log, it turned out there was many IO
errors on a particular disk.
Maybe it's a cause of disk IO errors causing the virtual memory
system to fail.
