Does [adding files from shell] work?

Yes - I explained that in the postscript. It would **appear** that Xcode is trying to add the file as cvs add path/file instead of (cd path ; cvs add file ) looking at the groomed output.

If command-line CVS works, then it's an Xcode-specific issue, and we can diagnose it from there. For example, what version of Xcode and OS
X are you using?

Yup, my gaffe - Xcode 3.1.2, 10.5.5, standard CVS. Is there log somewhere of the actual cvs commands sent, beside the SCM log?
