Sometimes cleaning targets or exiting XCode helps but often it doesn't.

I always either Build & Go (Debug) or Build (Cmd-B) and then Debug (Cmd-Y).

First thing to check: Make sure "Load Symbols Lazily" in the Debugging panel of the Preferences window is unchecked.
