not breaking on breakpoints

This is extremely annoying and I haven't been able to find a workaround. Searching the list archives is not working at the moment so I thought I'd bring it up here.

I have a few breakpoints set in my code. Sometimes they take and sometimes they don't. Either way, I see "GDB: Running" in the status line.

Sometimes cleaning targets or exiting XCode helps but often it doesn't.

I always either Build & Go (Debug) or Build (Cmd-B) and then Debug (Cmd-Y).

Is there a workaround?
