This is great in XCode but still the files in the actual filesystem mostly end up in either the project root or under 'Classes', depending on what was selected when the file was originally created.

It's easy to make Group 'Foo' point to a new directory but either the original files go red if I left them in the group or, if I drag them out first, then move them back in, they end up with path ../filename.

Have you tried using the repository management interface in Xcode? I think it will do most of what you want. The only thing it won't do is associate your groups with a filesystem path, which AFAIK you have to do manually. But that only matters for files you want to add to those groups.
Of course, all of this is 3.x stuff.
