I've taken existing templates and made several modifications. There's some useful info here <http://meandmark.com/blog/2011/11/creating-custom-xcode-4-file-templates/>, but most of it was a case of poring over TemplateInfo.plist files and working out what the various entries do.

It's a pity this isn't documented somewhere, but I'd hate to be the person doing it. And it's possibly a moving target; 4.2 broke some I did in 4.1.
