FYI and it if matters, at all I've got these devices:
Gen 4 iPod Touch - iOS 6.0 (10A403)
iPhone 4 - iOS 5.0 (9A334)
iPad 2 - iOS 5.0.1 (9A405)
iPad 3 - iOS 6.0 (10A403)
Just put Xcode 4.5.2 on a new account on one of the spare Macs.
If either of you (Matt or Tom) have a stripped down version of your offending project and want to email it to me, I can check if this is also reproducible on my end, and if it matters which device is plugged in or not.
If there is a new-er version of Xcode up on the devsite or in a β state, I wonder if this behaviour is also present in that version.
But let me know and I can at least run a quick test to see if I get the same problem.

If I switch it to the Simulator and use it, then close it, then open it again, we're back again at "iOS Device". It's maddening.

Not sure how much help this information is to you, but luckily it's worked for me (since this particular misfeature, minor as it sounds, is maddening in practice).
