Thanks for the clarification. I was revealing my ignorance of the whole
auto-save thing.
I agree. I've been saving or not saving my files for decades, and hardly ever
lost a thing. I don't need my work environment ruined for a feature that helps me.
So I turned auto-save off globally in
System Preferences > General > [√] Ask to keep changes when closing documents
To have it take effect in XCode I apparently had to reboot, not just quit and
relaunch. But now all seems good, so far. All my windows have their right
titles, several minutes later, which is at least a new record.

Xcode 3.x was never designed to work with Lion, so it's not surprising that if it collided with Lion features, Xcode would lose.
People have been able to work with 3.2.6 + Lion, and what you're reporting is unusual, so maybe there's a solution. What I'm saying is that you should be prepared to hear that the only solution is "don't do that."

Figured as much. Just asking just in case someone knew of a work around.
Not fatal or dangerous, just annoying. Oh well.
Note : it is the window title name, not the source file on disk itself.

Sometimes choosing "Clear File History" will reset it - but then, sometimes it doesn't.
I tried googling for "Clear File History" but can not find anything, except an indication it was removed from XCode 4. Meanwhile in XCode 3.2.6 I can't find Clear File History anywhere in the menus or in the list of items under Key Bindings.
I would love to try whatever this is to see if it helps at all.
