Xcode 4/5/subsequent? The IDE edits files, but in Xcode’s user model, that’s not the same as each file being a “document.” That was abandoned in Xcode 4, and a lot of people (including me) had reactions that began at “disconcerted,” and climbed sharply from there.

In the current user model, projects can be called “documents,” but for the contents of those projects, the IDE is a browser, including breadcrumb navigation. If you look at it that way, it’s consistent.
