I dunno, but I think I remember complaints about Xcode losing focus in a text editor repeatedly and annoyingly. But this was perhaps Xcode 6, and I don’t remember hearing anything about it recently. IIRC, this would depend on what text file you were editing, and how long it was (and perhaps what was shown in the assistant editor pane).
The other thing that springs to mind is that if you have multiple input devices connected, you might encounter something like this if the system keeps switching between the devices, each of which has its own idea of where the mouse is. Such behavior would in effect be a hardware problem.
I only have the one keyboard connected, and no mouse at all. Restarting VoiceOver has temporarily fixed it, but this only just started, so I expect it to return. At least I have a stopgap fix for the moment. Dealing with VoiceOver can be such fun!
