my concern for the blind developer would be more the Xcode environment itself than the device, but I say that as a non-blind developer. As for testing on the device itself, I have just tested VoiceOver for my own application and it would seem reasonably useful for a blind developer.
VoiceOver is definitely not available in the Simulator. It contains a different tool, the accessibility inspector, which in itself is not suited for blind developers (All you hear is "iPhone Simulator Window", regardless of which app is actually running).
My advice: Find a developer with a certificate in your area and just try it out, especially if the Xcode environment itself is accessible enough.
