I've also read that Smile can be used to create custom dialogs of more complexity than possible with display dialog, so perhaps that's a forth way to do it.

I can confirm that. The palette of the dialog components ("controls") of Smile, that you use to populate a new dialog by drag & drop, currently includes the following control kinds, that you then program ` la Hypercard, but in AppleScript:

----------------------
Push Button - Default Push Button - Cancel Push Button (activated by escape key) - Bevel Button (accepts dropped objects)
Static Text - Editable Text Box - Password Text Box
Popup Menu Button
Continuous Slider - Discrete Slider - Little Arrows
Radio Button - Check Box
Time Clock (adjustable) - Date Clock (adjustable)
Progress Indicator - Chasing Arrows
Horizontal Visual Separator - Vertical Visual Separator
Disclosure Triangle
PDF Holder - Icon Control - Image Well (accepts dropped objects)
List Box - Menu Group Box - Group Box
Tabs
----------------------

That's for OSX. On OS9 the palette is slightly less rich - that's Appearance vs Aqua.
