This is how I would do it in Photoshop (version 11): File -> File
info. And then the "Keyword" textarea under the first tab
"Description".
Background info: I am a user of Extensis Portfolio (version 9.0).
I'm a fan of Portfolio's powerful "Watched Folder" feature to
collect images and automatically adding them to a catalog and (last
but not least) extracting keywords from the added files.

Exiftool - a command line tool
It can be scripted from AppleScript via "do shell script" and has the
most consistent, broad support for various types of metadata and file
formats. I'm also an Extensis Portfolio user - if you need help you
can also catch me on their forums.
