Nonetheless, it's clearly not just .sdef files here. If I look in Console.app, I see several entries like this:
25/10/2015 10:43:52.499 PM Script Editor[10436]: [default] Failed to updated bookmark for item (null) [B19AD3BE-5BD4-4455-9858-EACC1F793D84] - URL:file:///var/folders/9d/n7rpchy88xlg3b008k8ydqlh0000gn/T/Mail.sdef with error Error Domain=NSCocoaErrorDomain Code=260 "The file couldn’t be opened because it doesn’t exist."
And:
25/10/2015 10:43:52.503 PM Script Editor[10436]: -[SFLListItem synthesizeMissingPropertyValues] Failed to updated bookmark for item (null) [0E8FF169-0D54-4185-B715-5C42F6E44357] - URL:file:///Users/shane/Library/Mobile Documents/com~apple~ScriptEditor2/Documents/Untitled.scpt with error Error Domain=NSCocoaErrorDomain Code=260 "The file couldn’t be opened because it doesn’t exist."
And I believe they represent the phantom entries. If I look at the .sfl file you mentioned, I can see the above paths listed, but with no data/bookmark value, whereas those that appear OK in the menu all have data/bookmark values in the file.
So I think it's a little more complicated...
