Only Dictionary I use is Excalibur ( 4.0.2 ):

It's free can be accessed without leaving the keyboard, spell checking/changing/adding new words/etc can usually be done without the mouse. Of course, it works in any app since it is a completely separate app. Fast, and did I mention it is FREE? ;-)

Looking at the list of dictionaries available, I see one for "Medical" and another for "Bio-med". There are about two dozen foreign languages, also. All I use is the html and "Standard" ones.

But I also don't use Word or AppleWorks...
