Let me rephrase. NeXTies and classic mac users have different ideas
of focus/frontmost.
Utility windows should never become the target of key commands. You
should never have to make a utility window frontmost in order to use
a key command (not press keys) on it. Example is in Photoshop, you
can make new layers without making the Layers window frontmost, and
clicking a palette/utility window does not make it respond to any key
commands.

That's nonsense. They should not "get focus" (you meant "to become first
responder", actually) _too often_ (like when you just click a button in them
or so), but they *HAVE TO* whenever you need to write to them.
Like Font panel: just for selecting anything from the browser there's no
need to become first responder, *BUT* when you want to _enter_ (not just
select) the font size, it has to become FR. The same applies for the IB
inspector: it should not become first responder so far as you only connect
outlets/actions or click other widgets there; it *HAS TO* become FR though as
soon as you need to write anything (like window title, or widget size)
