I'm trying to add a toolbar to a document-oriented app. I thought I was following the sample code pretty much exactly, but the sample code works and mine doesn't.

Hi all,

I just figured out what's going on...

In a previous run, NSToolbar had autosaved a configuration of zero toolbar items to my app's plist in ~/Library/Preferences

So, it was reading that configuration I guess. Deleting the plist from my Preferences folder caused the toolbar to appear on my next run.
