I can give a partial answer for this, the two-part question that is the first question #2 in your message. We'll call these questions 2.1.a and 2.1.b. :->
To make the graphics side of the appearance, you might check out Aquatint at http://www.sticksoftware.com/software/Aquatint.html. For a demonstration of how to make a custom control using images generated by Aquatint, check out http://www.sticksoftware.com/software/CircularSlider.html, an open-source project implementing a custom control. Note both of these projects were written by me; my apologies for the plug, I'm really just trying to be helpful.
