What is a low cost way of making an application icon?

Self-promotion:

Icon Machine lets you create and edit icons, including defining the transparency. You get the best results if you create the image in Photoshop and then import it, but Icon Machine also has enough built-in drawing tools to get the job done. It's $25 shareware.
