Some more information that might help.
I use file vault
It only happens on the admin account (Which was the only acct on the computer until tonight.)
I created a new user account and tried the app and the document icons work.

Hmm well it is copied into the resources fold and it is an icns file I rebooted and nothing seems to help. I even did as one person suggested and copied the app to the desktop. The application icon works fine, it's the document icons for the files that the app creates.

Mark.

Ok, well I have created document icons for and application I am
working on and have listed them as the icon in the target info but
the icons are not showing up in the finder. What do I need to do to
make sure that my app's icons are shown?

-> Make sure they're .icns files and not .tiff or any other image format. -> Make sure your icon has been added as a bundle resource to the built application. -> Make sure you correctly typed the name (also watch out for upper/ lowercase, or your app'll break on case-sensitive HFS or UFS like Photoshop does). Don't include the suffix. Your icon should show in that example box next to the icon file name entry field. (for an app icon) -> Log out and back in or duplicate your app's bundle in the Finder to get Icon Services to reload your app's icons
