In Carbon, you often have to implement the scroll wheel yourself for many UI elements. This will lead to small variations like the one you describe.
FWIW, iTunes' scroll bars are custom controls anyway (especially obvious in version 7, since they are skinned differently).

If I try the same thing with the scrollbar in iTunes it will not work. only when the mouse is entirely in the tree view outside the scrollbar does the scrollbar function.
