I agree the guidelines aren't perfect, nor completely up-to-date with
some of the newer interface elements, but I still think there's a
reason to stick to them -- or if not them, what "feels right." Will
a majority of long-time Mac users think that having a "Select all"
button actual be the first row in a table feel right?
But about your iTunes example, I'm curious... is that in iTunes 7? I
just went to try to look for it, but I couldn't really find a table
view with an "all" row at the top.

Why not just forget having the special row in the table, and
instead have a

you're more likely to confuse your users than anything else.
I agree that this is an area of contention to some (I can't say I've
ever personally used an "All" row ... ever ...), but ...
See http://developer.apple.com/documentation/UserExperience/
Conceptual/OSXHIGuidelines/XHIGIntro/chapter_1_section_1.html for
reasons why it's a good idea to stick to Apple's Human Interface
Guidelines,
and the "List Views" section at
http://developer.apple.com/documentation/UserExperience/Conceptual/
OSXHIGuidelines/XHIGControls/chapter_18_section_7.html for
exactly what the Guidelines say about table views.
... citing the HIG(uidelines) as *the* reason why you shouldn't do
it doesn't make much sense. For one, they're guidelines and aren't
always right. For another, Apple, who wrote the guidelines, use the
"All" approach in table views in iTunes. In that case, it's hard to
imagine how they'd have done it more cleanly (because they're not
necessarily source lists, there's more than one, and they're all
likely to have many items in them) to make it easier to distinguish
"one or more selected but not all" and "all selected".
So, in short, the guidelines aren't perfect. :-)
