I agree the guidelines aren't perfect, nor completely up-to-date with

Unless I misunderstood, he's not asking for a button in the first
row. Just a row. As to long-time Mac users ... if we're talking
pre-OS-X users, then that's a whole different discussion for another
list but suffice to say they (not "we", I'm new to Macs as of OS X
10.0) have quite a lot of negative opinions of the OS X UI in general.
This is a new OS with new idioms. If a new concept calls for a new
control or approach, as long as it's clean and simple, it'd be
ridiculous to avoid it and try to make the existing basic controls
work.
Personally, again, I don't like the idea of an "all" row in most
situations. In iTunes and similar situations, it's the only way I see
a filtered list series with "show all" working well.
But about your iTunes example, I'm curious... is that in iTunes 7? I
just went to try to look for it, but I couldn't really find a table
view with an "all" row at the top.
Yep. And every version prior. In iTunes 7, however, you see this by
using List view and turning on the Browser (Cmd-B) to show Genre,
Artist, and Album.
