Yes, it does. I prototyped an inkjet driver for one of the printers I write
drivers for to take CMYK directly, and it seemed to work fine. A CMYK image
printed from Preview resulted in those exact CMYK values being passed to my
driver.
As to when you will see inkjet drivers taking CMYK directly, I wouldn't hold
my breath. That won't happen until printer vendors give up sRGB as a "good
enough" color space and allow users more flexibility in how color is
reproduced.

There are no printer drivers with custom paper size support because it's an
OS limitation. Hopefully, this will be addressed in Jaguar.
