Dear Kurt,
I would suggest that you look in the Medical Department of a good local
University. I myself was able to go through extended visual testing
(including the test you mention) at the University of California at
Berkeley.
From looking in their Web site, I see that the University of Michigan has a
Medical School, which in turn has an Ophthalmology and Visual Sciences
department: that is where they may be able to administer those same visual
tests to you.
If you need assistance, ask Dr. Wayne Verdon (of UC Berkeley, the person I
consulted) to refer you to a good center in your area, and he may be able to
help. His office number is (510) 642-2020.
