Yes, it is a bit of a pain that CMYK Optimizer Server does not handle
greyscale.
So have a look at PuzzeleFlow from AC&C HSH Group in Poland.

This is a PDF workflow tool which can be tailored for many needs and scale,
most interesting is that CMYK icc profiles, the K element, can be applied to
greyscale images.
The prices look very completive
