It looks to me that CS5 on our new Mac Pros is doing the "unsafe" save.
We don't use third party server software here and today's examples aren't using OS X Server either -- we're reading a PSD from another Mac on the network, making a couple of edits and then hitting Command-S.
The reasons that I suspect that PS CS5 is implicated are:
1. We don't get this happening with any other software
2. Photoshop's log states clearly that it has saved the file incorrectly:

The CS4 Edit Log from the local copy of Photoshop running on the Mac that the two Mac Pros are saving to returns nothing if you do the same search -- and it's log is around 50MB in size.
If third party software is to blame how are those save entries in Photoshop's log being generated?
