The format flags in your ASBD are invalid. Give this a try:
theFormat.mFormatFlags = kAudioFormatFlagIsBigEndian |
kAudioFormatFlagIsSignedInteger | kAudioFormatFlagIsPacked;
...also don't forget to change the file type parameter of
ExtAudioFileCreateNew to kAudioFileAIFFType
