I'm wondering particularly because most examples seem to use the
default output device, but the source for Adium uses the system
output device

A justification for using the default system output might be that
your creating sounds that are "Alert-like", for example the old
SysBeep() call always uses default system output. If your playing
music however, you should definitely be going through the default
output device.
And I'm curious if it could be causing problems in some systems
with sound configurations that include both internal speakers and
external usb-based sound devices.

If there is an issue it's not likely related to selection of default
vs default system output. Think of it this way - if you swap the
devices in System Preferences does the issue still occur? If yes,
then it's an issue with the application, if not then it's probably an
issue with the device itself (in which case you might start seeing
issues playing music and from other apps that use default output).
