I took a hiatus from this issue for a while, but lately more and more of our customers have started reporting it so I've had to revisit it.
The choppy/stuttering audio only happens on Mac OS X 10.5.7+ (including 10.6.x). The specific mac doesn't matter (I've gotten it to happen on iMac PPC, intel MacBooks, intel MacPros, etc).
I don't think it's a hardware issue. Or, rather, if it is a hardware issue, something introduced in 10.5.7 is provoking the hardware. That said, it will occur across a wide range of USB headsets. I happen to have one that can reproduce the issue (somewhat) quickly, but it's occurred with every headset I've tried it with (given enough time).

As far as I can tell, the API calls I'm making are valid. I'm not sure one what I can do to code around this problem.
