Just had a few moments to try it out and I think I figured out the
issue. In the past I had been trying to install Coolkey somewhere
that made sense from an OS X perspective, namely under /Library/
CoolKey or ~/Library/CoolKey. When I instead placed it under /usr/
local (e.g. /usr/local/lib/pkcs11/libcoolkeypk11.dylib), it
successfully loaded the module and I was able to connect to a website
that required CAC.
Using Firefox 3.0, Mac OS X 10.5.2, and the binary download link in
the message below.

Where did you get the Coolkey binary? Did you compile it yourself (on
Leopard or Tiger?), download it from the Coolkey wiki, or somewhere
else?
