How it's implemented depends. Are you using the Apple S/MIME implementation or another certificate provider?
At DOE we use Entrust with the Good MDM at some of our components. We are loading the Entrust PKI credentials on the iOS device (mostly iPads, but some iPhones). Up to now it's been a manual process to load the credentials, but Good is working on some upgrades (they may be out by now; I'm not sure) that automate the process a little more.
