Here's what the man page says:
-rv This option indicates that reverse video should be
simulated by swapping the foreground and background colors.
+rv Disable the simulation of reverse video by swapping
foreground and background colors.
If that's not what is happening, I suggest you mention it to Thomas
(CC'd).
