FWIW, I've noticed very similar behavior using exmh, a Tcl/Tk email
reader. The first time I read email with a given non-ASCII encoding in
a given session, there's a hiccup while a whole bunch of font info gets
extracted. It's tolerable across my local LAN, but un-fun when I'm
ssh'd in from a hotel or someplace. There's probably nothing to be done
about it from the server side. I might look into fontconfig ...
