cmd-v isn't used by X11.app. You need to paste however your application wants you to paste. Middle click (PRIMARY) or control-v (CLIPBOARD).

You have that KVM, too? My condolences. I have the 4-port one. I've noticed that it occasionally sends random key-presses that I don't issue. I have this problem with ESC which is highly irritating and problematic.
Try moving the keyboard from the "keyboard" USB slot to the general USB slot on the back. You won't have keyboard hotkeys for swapping, but it makes things behave properly.
