I fought this problem for months back when I was engineering a Mac-based
contextual intelligence system. It seems to crop up whenever certain
sort of old-style Mac things are done. Starting System Events, for
instance, or running sips to generate a Finder icon of a document.
My impression (from a couple of years ago, and mainly with 10.5) was
that the image and video processing subsystem of OS X was stealing focus
momentarily when it had to generate certain kinds of images, and that
QuickLook was responsible for most of the focus stealing I was seeing.
If you uncheck "show icon preview" and disable folder actions in Finder,
it should help.
