That's not something Apple or XQuartz can fix; X11 programs, especially the older Xt-based ones, are annoying that way. You might be interested in http://blog.ssokolow.com/archives/2011/05/01/modernizing-xterms-paste/ though.

Interestingly, the only way that I can use my MacBook for serious work is "the old way". I need Focus Follows Mouse and Select - Paste using middle button (Option-trackpad click, or mouse with middle button mapped to thumb button). Cmd-C and Cmd-V drive me crazy!
While I have come to terms with the trade-offs, I wish that the rest of MacOS could be configured "the old way". I realize that Focus Follows Mouse could be messy due to the Aqua application menu bar.
I mainly copy and paste between xemacs and xterms, but also browsers which is a bit troublesome since I often forget to Cmd-C in the browser, but the other direction is smoother with all of the X11 Pasteboard Preferences selected.
If there is something that I am missing, please let me know.
