Beginner Question: Internal error

When I press 'Info' icon in Toolbar, the internal error alert sometimes comes out. The content is the following:

Uncaught Exception: Invalid parameter not satisfying: (index >= 0) && (index < (_itemArray ? CFArrayGetCount(_itemArray) : 0))

Stack Backtrace:
The stack backtrace has been logged to the console.


Did I do anything wrong?
