It looks like Xcode is caching too much.
XCode has a host of caching-related bugs. AFAIK, bug reports have been
filed by various people, but the behaviour persists through every
version, so I wouldn't hold your breathe on a quick fix. :/
For the most part, I find any file-caching problems can be solved by
simply viewing the file(s) in question and saving them again... most of
the issues I've seen are clearly time-stamp related, and this thus
fixes the problem. Of course, it's impractical when the problem occurs
across many files, in which case restart XCode may be faster - but I've
had the rare case where even this wasn't enough. :(
Is this a known bug or is it worth filing a bug report?
Well, that's always an interesting question. ;)
I'd say if you can pin-point an exact sample scenario, file the bug
report. I can't seem to pin down a reliable cause myself, so perhaps
the problem persists because Apple can't either.

P.S. [Entirely subjectively] I've had more cache related bugs with
indexing off. For me on a "mere" 800mhz iBook indexing is not an
option, but if you've got a more powerful machine, and can put up with
the performance hit, you might find this solves you problems, in most
cases.
