Well, that's always an interesting question. ;)

I'd say if you can pin-point an exact sample scenario, file the bug report. I can't seem to pin down a reliable cause myself, so perhaps the problem persists because Apple can't either.

Apple's official word is to always file bug reports on everything ever. Even if they're closed as duplicates, the number of duplicates for a bug are considered pseudo-votes.
