I don't think this is necessarily specific to Xcode.

At some point post 10.4 (10.4.1/10.4.2) PhotoShop exhibits the identical behaviour.

I have trashed launchservices caches, rebuilt the ls db via:

/System/Library/Frameworks/ApplicationServices.framework/Frameworks/LaunchServices.framework/Support/lsregister -kill -r -domain local -domain system -domain user

restarting, etc with no improvement in behaviour. Nothing written to console.

Since it finds the app to launch it, but fails to open documents dropped on the running application's icon, something might be going south with the odoc appleevent construction or emission.

I know. File a bug, but it's all so vague.
