And I don't think there's a real-time priority that iTunes can assert,
either, so its disk requests get in line with everyone else's.  Is there an
iTunes read-ahead buffer length that can be increased?

I suggest using Activity Monitor to observe Disk Activity.  When a stutter
is heard, glance at the Activity Monitor window.  It will take several
seconds for the burst to disappear from the scrolling "trace".

Another possibility is to figure out how to get 'top' to record its output
to a file, so it can be analyzed later.  Of course, the file being recorded
contributes to disk activity.
